Sunrise Retirement's primary health care facility is a nursing care program that is divided into a number of smaller, specialized care units to ensure personalized

attention and quality care.  Located in the Health Center, our licensed nursing care services include a 24 hour professional nursing staff, medication administration by licensed personnel, supervised diet, and planned programs and activities.  Our active rehabilitation department allows residents to progress through levels of care to maximize their independence.  Sunrise is very capable of meeting a resident's

individual needs through its offerings of therapies, hospice, and dementia care services.

BASIC SERVICES:

	24-hour professional nursing staff.
	Medication administration by Licensed Nursing Personnel.
	Three meals per day served family style in the dining room.
	Two assisted whirlpool baths weekly.
	Physical therapy for rehabilitation or health enhancement.
	Therapeutic recreational program
	Weekly housekeeping services and laundering of linens.
	Social and recreational activities.
	24-hour emergency call system

COMPREHENSIVE CARE:

 

IN CONJUNCTION WITH BASIC SERVICES, COMPREHENSIVE NURSING FACILITY CARE ALSO INCLUDES:

 

	Bedfast or chair fast bathing.
	Partial incontinence or incontinence care.
	Assistance with eating.
	Increased need for supervision/assistance due to psychosocial or physical changes.
	Special treatments as directed by physician.
	Special diets as directed by physician.

 

ADDITIONAL SERVICES:

	Respiratory treatments:$7.50 per day
	Beauty and Barber services: See cost sheet
	Laundry Marking service:$25.00 one time fee.
	Laundering of personal clothing:$30.00 per month.
	Nursing supplies such as: syringes, dressings, lotion, Kleenex, etc.
	Van Service: $20.00 one way; $25.00 round trip.

Nursing Facility Medicare Skilled Nursing Program

 

GOAL: To provide quality nursing and rehabilitative services to qualified individuals, assisting them to attain their highest level of independence.

 

SERVICES:

 

Part A:

             Room and Board during qualified days.

             24—Hour Professional Nursing Staff.

             Medication administration by Licensed Nursing Personnel.

             Physical, Occupational and Speech Therapy Services.

             Three meals per day including supervised diet service.

             Therapeutic Recreational Program.

             Housekeeping, Laundry and Linen Service.

             Partial or Total Assistance with Activities of Daily Living.

             Special Treatments as ordered by your physician.

             Furnished Rooms.

             Chaplaincy Services.

             Social Services.

             Pharmacy Services.

             Transportation Services.

             Home assessment prior to discharge, as ordered by your physician.

             Durable Medical Equipment needed during your days with us.

 

Part B:
